Participation Requirements

INHALE participation requirements have been categorized into four groups:

• Requirements for the
  • Continuing and Onboarding Physician Organizations 
  • Continuing and Onboarding PCPs/SCPs

NOTE: VBR Eligible Providers (PCP/SCP) are pulled from the INHALE Admin Portal. It is the sole responsibility of the PO to maintain/update/verify the lists of providers participating and to inform INHALE if a change in status is needed,(i.e. removing provider). INHALE does not/will not add people to the list of participating providers outside of recruitment windows (Spring/Fall).

Updates to participation requirements may occur at any time and all participating POs and physicians will be given adequate notice of changes.  All questions should be directed to the coordinating center.
